Membership Guidelines & Exclusions:
- Membership fee is due at the time of enrollment, valid for one year, and is non-refundable.
- Membership is only offered to those without dental insurance and government funded insurance, and cannot be combined with dental insurance.
- Membership cannot be used with third-party financing.
- Membership discounts cannot be combined with any other offer or discounts.
- No refunds will be given at any time if not utilized or for failure to schedule or maintain appointments.
- Discount is applicable only when full payment is received on the day of treatment with active membership.
- Only applicable to services performed at Plexus Dental. Specialist referrals will not honor the discount.
- Does not apply to costs covered by worker’s compensation or covered under auto or medical insurance.
- When used for clear aligners like Invisalign treatment, the person being treated must remain a member for the entire duration of treatment.
- Plexus Dental reserves sole discretion to refuse treatment to any plan member or dismiss any plan member as a patient.
